Travel Requirements of LGUs Around the Philippines during Community Quarantine

This is a consolidated list of the travel requirements of different local government units (LGUs) around the Philippines. This is last updated on 13 October 2020. Please take note that requirements may change from time to time. We highly suggest contacting the receiving LGU before traveling for a smooth journey.

During your travel, make sure to strictly comply with the health and safety requirements set by the government.

  • Wear a facemask and face shield properly all the time throughout your journey.
  • Follow the social and physical distancing measures at airports, buses, seaports, and other public places.

Luzon

Travel Requirements to Baguio City

Baguio currently allows the following people to enter the city

  • Returning residents
  • Returning workers/employees
  • Essential travel
  • Official business/work travel
  • Tourists from Region 1

Before traveling, please register online: hdf.baguio.gov.ph at least 3 days prior to your travel date. Please refer to the following matrix for the requirements.

OFWs going home to Baguio should register online at hdf.baguio.gov.ph and upload your Clearance from the Bureau of Quarantine at the airport as well as your proof of residence in Baguio.

Tourists

As of this writing, only tourists from Ilocos Norte, Ilocos Sur, La Union, and Pangasinan are allowed to enter Baguio City. Only 200 tourists will be allowed per day with no underlying health conditions and no COVID-19 symptoms and exposure.

Travel Requirements to Puerto Princessa City

LSI

  • Medical Clearance Certificate recently issued by the City/Municipal Health Office
  • Travel Authorization from PNP
  • Proof of residency in Puerto Princesa or endorsement from the city

ROF

  • Negative result of RT-PCR test
  • POEA Certificate / Passport
  • Government ID or any valid ID showing proof of residency in Puerto Princesa

APOR

  • Negative result for RT-PCR test valid within 3 days prior to arrival in Puerto Princesa
  • Mission Order for AFP Personnel; Notice of Work Assignment; Company Official Travel Order
  • Endorsement from the City Emergency Operation Center

Please take note that all travelers are advised to coordinate with the Puerto Princesa City EOC at 0965-314-8399 or at 0938-794-4004 once they have all the required documents.

Travel Requirements to Ilocos Norte                  

Returning Overseas Filipino (ROF) and Locally Stranded Individual (LSI) Requirements

  • Negative RT-PCR result dated within the last 72 hours
  • Accomplish Request for Entry of Returning Resident Form
  • Health Declaration for Returning Residents
  • Medical Clearance Certificate issued by City/ Municipal Health Office or DOH-accredited hospitals provided that the returning resident is not a contact or suspected COVID-19 case OR if was confirmed COVID positive, must have completed 14-day isolation with Certificate of Completion from the accredited facility)
  • Travel Authority for Returning Residents issued by JTF-CV Shield/ Philippine National Police in their locality        

Authorized Person Outside of Residence (APOR) Requirements:

  • Negative RT-PCR result dated within the last 72 hours
  • Accomplished Affidavit of Undertaking
  • Company ID or Travel Order, Business Order, Mission Order, proof of work in Ilocos Norte, etc.
  • Provide complete personal information (eg. Employee’s name, contact number, company contact name and number, and duration of stay in Ilocos Norte)

Indispensable Travel (IT) Requirements:

  • Negative RT-PCR result dated within the last 72 hours
  • Proof of Indispensable Travel (such as family emergency or medical emergency)
  • If staying at Ilocos Norte, must complete mandatory facility quarantine
  • Medical Clearance Certificate issued by City/ Municipal Health Office or DOH-accredited hospitals provided that the returning resident is not a contact or suspected COVID-19 case OR if was confirmed COVID positive, must have completed 14-day isolation with Certificate of Completion from the accredited facility)
  • Provide complete personal information (eg. Employee’s name, contact number, company contact name and number, and duration of stay in Ilocos Norte)

Required documents must be sent to ilocosnortecovidcenter@gmail.com at least 3 days before the flight date and passenger must bring a hard copy to expedite entry. 

Tourists

Ilocos Norte has reopened to visitors from the Ridge and Reef Travel Corridor composed of Ilocos Sur, La Union, Pangasinan, and Baguio City on October 1, 2020. Below are the steps to follow if you are visiting Ilocos as a tourist and the documents required.

Travel Requirements to Basco, Batanes

  • Medical Certificate from the City/Municipal/Provincial Health Office of the LGU of Origin
  • Travel Authority from the PNP of the LGU of origin
  • Optional Negative Rapid Antibody Test (RAT) or RT-PCR Test

Visayas

Travel Requirements to Cebu City

ROF

  • Negative RT-PCR result
  • Quarantine Certification issued by DOH- Bureau of Quarantine
  • Identification Card
  • Travel authority issued by the Joint Task Force (JTF) Covid (CV) Shield

LSI

  • Letter of Coordination from their Barangay of residence in the City of Cebu
  • Medical Certificate

ALL PUBLIC APOR

  • Travel or Mission Order issued by Head of Agency;
  • Certificate of Employment (except for uniformed personnel);
  • Valid ID

LONG-TERM PRIVATE APOR – APOR staying in Cebu City for more than 14 days

  • Travel Clearance from the Office of the Cebu City Mayor
  • Letter of Coordination from their Barangay of Rresidence in the City of Cebu except for Government Agencies, Local Government Units and Government Personnel
  • Valid ID
  • Medical Certificate

SHORT-TERM PRIVATE APOR – Non-resident APOR travelling to Cebu for less than 14 days

• Travel Clearance issued by the Office of the Mayor of the City of Cebu

• Negative RT-PCR Test Results within 3 days prior to departure

• Return Flight Ticket – Failure to provide Return Flight Ticket will deem such individual as a long-term APOR and the protocls for long-term APORs shall apply

• Valid ID

Travel Requirements to Lapu-Lapu City

ROFs and Returning OFWs 

  • Negative RT-PCR result and Quarantine Certification issued by DOH- Bureau of Quarantine
  • Identification Card
  • Travel Authority issued by the Joint Task Force (JTF) Covid (CV) Shield

LSI

  • Medical clearance certification issued by the City/ Municipal Health office based on the following: That the LSI is neither a contact, suspect, or probable or confirmed COVID-19 case; and That the LSI completed the 14-days quarantine based on quarantine standards set by DOH; or That the LSI confirmed as a COVID-19 case was tested negative through RT-PCR twice
  • Travel Authority issued by the Joint Task Force (JTF) Covid (CV) Shield
  • Valid identification Card

APOR

  • Certificate of Employment
  • Company ID
  • Round-trip ticket
  • Letter of Appointment / Proof of visit

Tourism

  • Confirmed DOT Accredited Hotel/ Resort Booking within the City of Lapu-Lapu or Island of Mactan
  • Negative RT-PCR Swab test (72 hours prior departure) from DOH accredited laboratory
  • Round trip ticket 

Travel Requirements to Mandaue City

ROFs and Returning OFWs 

  • Negative RT-PCR result and Quarantine Certification issued by DOH- Bureau of Quarantine
  • Identification Card
  • Travel Authority issued by the Joint Task Force (JTF) Covid (CV) Shield

LSI

  • Medical clearance certification issued by the City/ Municipal Health office based on the following: That the LSI is neither a contact, suspect, or probable or confirmed COVID-19 case; and That the LSI completed the 14-days quarantine based on quarantine standards set by DOH; or That the LSI confirmed as a COVID-19 case was tested negative through RT-PCR twice
  • Travel Authority issued by the Joint Task Force (JTF) Covid (CV) Shield

APOR

Travel Requirements to Bohol

  1. Negative Reverse Transcription Polymerase Chain Reaction (RT-PCR) Test result.
  2. 14-day Quarantine Completion Certificate issued by the MHO/CHO of the LGU of origin.
  3. Undertaking by the returning constituents that they agree to be quarantined and to undergo all required medical tests and processes.

Travel Requirements to Iloilo

LSIs and ROFs

  • Notice of Coordination and Acceptance through the Iloilo City Emergency Operations Center (IC-EOC)
    • Notice of Coordination to be submitted prior to arrival to ports of entry in Iloilo City and shall indicate whether they will be fetched by private transportation or if such an arrangement will not be feasible.
  • 14-day quarantine upon arrival subject to the provisions set:
    • The option of submitting themselves for a 14-day quarantine in quarantine facilities provided by the city; or 
    • Observe strict 14-day home quarantine (under monitoring by BHERTs)
  • RT-PCR test shall no longer be mandated by the city government. LSIs and ROFs may voluntarily submit themselves to the same at their own expense.
    • In the event they obtain a negative RT-PCR test result, they shall be deemed as having completed their 14-day quarantine.

APORs

  • Will be staying in Iloilo City for less than seven (7) days
    • Must indicate to Iloilo City Emergency Operations Center (IC-EOC) their temporary place of abode within the city
    • No RT-PCR test nor 14-day quarantine is required
  • Will be staying in Iloilo City for more than seven (7) days
    • Must indicate to Iloilo City Emergency Operations Center (IC-EOC) their temporary place of abode within the city
    • Must undergo strict 14-day quarantine therein or submit themselves for RT-PCR testing
    • In the event they obtain a negative RT-PCR test result, they shall be deemed as having completed the 14-day quarantine

Travel Requirements to Bacolod

LSIs

  • Philippine-issued ID card showing the name, photo, address, and signature of the returning resident
  • Contact numbers and specific address in Bacolod City
  • Medical Certificate from City/Municipal Health Office (of your current location) or any DOH-acredited facility stating that you are fit to travel and does not manifest any symptom of COVID-19 PNP
  • Travel Authority for LSIs coming from Cebu and Mindanao

Travel Requirements to Aklan

The requirement for Negative RT-PCR result for Locally Stranded Individuals is no longer imposed as a pre-departure requirement. However , upon arrival, LSIs will have to be swabbed for RT-PCR test at the expense of the government before they are released to theur respective LGUs for a mandatory 14-day quarantine.

The following requirements need to be presented before boarding:

  • Travel Authority issued by Joint Task Force COVID Shield
  • Notice of Coordination or Letter of Acceptance issued by the receiving LGU (municipality/city)
  • Medical Clearance Certificate

Travel Requirements to Boracay (Caticlan)

Tourists

  1. Health Declaration Card which can be accessed via aklan.gov.ph.
  2. Confirmed hotel or resort booking on the island. You may book via Booking or Agoda for discounted rates.
  3. Negative RT-PCR test within 48 hours prior to the departure date.
  4. Email a copy of your Health Declaration Card (HDC), RT-PCR test result, confirmed booking slip, and proof of identification to touristboracay@gmail.com.
  5. As soon as the processing is successful, a copy of the HDC with Tourist QR code will be sent back to you via email.
  6. Present the HDC with QR code to the authorities in the borders for scanning.

Destinations in PH that Allow Tourists under CQ + List of Requirements

Travel Requirements to Dumaguete

  1. Medical Clearance Certificate issued by the City/Municipal Health Office of the place of origin
  2. Travel Authority issued by the JTF CV Shield (PNP)
  3. Negative RT-PCR test result (Valid 72 hours after test result release date)
  4. Certificate of Acceptance from the City of Dumaguete5
  5. Must undergo 14-day quarantine upon arrival in a facility designated for the purpose.

In addition, all LSIs (Dumaguete residents) and workers MUST coordinate with the Local Government Unit at least 7 days before entering the City to allow the City Tourism Office to:

  • Validate barangay residency
  • Arrange to pick up at the port of entry
  • Accommodate at government-designated facilities for a mandatory 14-day quarantine
  • Prepare Certificate of Acceptance

Travel Requirements to Siquijor

Locally Stranded Individuals 

  • Barangay Certification of their home residence in Siquijor Province
  • Endorsement Letter from the LGU
  • Barangay Certification from the Barangay where they presently reside which certifies that he/she is an LSI, not a PUM or PUI, and that he/she underwent a mandatory 14-days quarantine
  • Updated Medical Certificate of good health and fit to travel from any government hospital or health center
  • Travel Authority issued by the RJTCV Shield / PNP Office of the place of origin
  • Coordination Permit from Siquijor Provincial Task FOrce COVID-19 through the Provincial Disaster Risk Reduction and Management Office.

APORs

  • Copy of Office Order / Request Letter that the APOR need to report from work since their service is essential for the company
  • Copy of valid ID as proof of employment of the said company
  • Request Letter addressed to the Provincial Governor
  • Updated Medical Certificate of good health and fit to travel from any government hospital or health center
  • Travel Authority issued by the RJTCV Shield / PNP Office of the place of origin
  • Confirmation of Availability of Quarantine facility
  • Coordination Permit from Siquijor Provincial Task FOrce COVID-19 through the Provincial Disaster Risk Reduction and Management Office.

Travel Requirements to Tacloban

  • Mandatory 14 days quarantine at designated Quarantine facility
  • Medical Certificate from DOH-accredited hospital or health center of their place of origin (not PUM/PUI)
  • Certification to undergo a medical examination as may be deemed necessary

Mindanao

Travel Requirements to Davao

  • Davao QR Code (https://hdform.davaocity.gov.ph/form) containing a health declaration form for contact tracing purposes. This must be scanned and filled out upon check-in at the airport of origin.
  • Medical clearance certificate from the city or municipal health office.
  • Travel Authority issued by the local police.
  • Certification – an undertaking of 14-day quarantine (at LGU quarantine facility/home)
  • Negative RT-PCR test result taken within 24 to 72 hours from the date of departure. Please take note that is NOT mandatory upon check-in at the airport of origin but will be required upon exit at Davao International Airport (DIA).
    •  A negative RT-PCR result taken within 24 to 72 hours from the domestic departure date conducted by a laboratory overseas for international passengers will be accepted. All data therein must have an English translation for non-English speaking countries.
    • Children 12 years old and below are exempted from the RT-PCR requirement upon check-in, provided that, the accompanying adult passenger presents negative RT-PCR results taken within 24 to 72 hours from the date of departure
    • All those who are allowed to board for Davao-bound flights without RT-PCR testing shall be tested upon arrival at the DIA and will not be allowed to exit the airport without a negative result.
    • All those who were tested in their place of origin within 24-72 hours of departure date but have not received their result shall not be allowed to exit the DIA until their test result is available
    • The negative RT-PCR result should be taken within 24 to 72 hours from the departure date. If the result is more than 72 hours, we shall re-test the passenger in the DIA 
  • The City Government of Davao WILL DISPENSE WITH AND WAIVE the issuance of a Letter of Acceptance or Notice of Coordination for Locally Stranded Individuals (LSIs), Returning Overseas Filipinos (ROFs), Overseas Filipino Workers (OFWs), and all other travelers to Davao City.
  • Certificate of Acceptance by the Receiving LGU is Not a Requirement on the management of LSIs.
  • Land travelers going to other Local Government Units (LGUs) passing through Davao City shall be escorted from one border to another. Airplane passengers who are not residents of Davao City arriving at the Davao International Airport (DIA) should coordinate with their destination LGU for the required documents and arrange their transportation or LGU pick-up vehicle.
  • Strictly observe a home or facility quarantine upon arrival in Davao City or nearby LGUs
    • For DAVAO CITY RESIDENTS – mandatory 14 day home quarantine
    • For APOR – observe work to hotel arrangement
    • For DAVAO CITY RESIDENTS SWABBED AT THE DIA – you will be placed in the city’s Holding Facility and allowed to go home only when you receive your negative test result in 24-48 hrs. A 14 day home quarantine is required.
    • For NON-DAVAO CITY PASSENGERS SWABBED AT DIA – please arrange for your LGU to pick you up RIGHT AFTER swabbing. Results will be sent both to the passenger & LGU through email.

Travel Requirements to Cagayan de Oro City

We do not have enough information for the travel requirements to CDO. Please contact your destination LGU for the updated requirements.

Travel Requirements to Surigao del Norte

  1. Returning Surigaonons. This includes all RESIDENTS (whether APORs or NON-APORs) of the Province of Surigao del Norte either by birth or by choice. Under Provincial Ordinance No. 07, s. 2020, Returning Surigaonons shall be subjected to:
    • Rapid Antibody Test
    • Fourteen (14) days Quarantine

Note: This rule does not apply for Returning Surigaonons, who will come to this Province through Siargao Island Airport or Surigao City Airport wherein a Negative Test Result on RT-PCR taken within 48 hours is required to be presented before boarding the plane and upon arrival at either Siargao Island Airport or Surigao City Airport. In addition, 14 days qurantine is still required.

  1. Non-Surigaonon Authorized Persons Outside Residence (APORs). This includes all NONRESIDENTS of the Province of Surigao del Norte, who are either WORKING (as government employees or workers in the private sector including the practice of profession) or DOING BUSINESS inside this Province. Under Provincial Ordinance No. 09, s. 2020, all Non-Surigaonon APORs shall:
    • Present NEGATIVE TEST RESULT on either Rapid Antibody Test or RT-PCR Test taken within seven (7) days prior to the date of arrival in Surigao del Norte
    • Undergo a MODIFIED SELF-QUARANTINE for not more than fourteen (14) days

Note: This rule does not apply for Non-Surigaonon APORs who will come to this Province through Siargao Island Airport or Surigao City Airport wherein a Negative Test Result on RT-PCR taken within 48 hours is required to be presented before boarding the plane and upon arrival at either Siargao Island Airport or Surigao City Airport. In addition, MODIFIED SELF-QUARANTINE for not more than fourteen (14) days is still required.

  1. Tourists. This includes all persons who are not Returning Surigaonons and Non-Surigaonon APORs. Under Executive Order No. 20-026, s. 2020, all TOURISTS shall present the following documents:
    • NEGATIVE RESULT ON RT-PCR TEST taken within forty-eight (48) hours prior to the date of arrival in Surigao del Norte;
    • Confirmed Booking of at least 15 days from the resort, hotel, or hostel duly accredited by the Department of Tourism as a quarantine facility for tourists;
    • A printed copy of “Provincial Health eForm” which can be generated from this link: https://covid19.ssct.edu.ph/healthform
    • Valid Identification Card
    • Shall only board accredited public utility vehicles/public vessels going to accredited Quarantine Facility;
    • Shall undergo 14 days Quarantine.

Travel Requirements to Butuan City

  • Medical Clearance Certificate
  • A declaration to undergo health check and isolation (14 days) upon arrival
  • Travel authority for Uniformed personnel / Government officials and employees
  • Must be tested negative at point of origin
    •  The negative test result must be presented and must not be older than five (5) days prior to their travel date
    •  It may be a Rapid Antibody Test (RAT) or RT-PCR test

Travel Requirements to Zamboanga City

Travel Requirements for entry to Zamboanga City via land, air, and sea travel:

  1. Barangay Clearance
  2. Medical Certificate (issued by the local health office)
  3. Travel Authority – JTF CV Shield (issued by the local Police office)
  4. Recent RT-PCR TEST yielding a negative result ( if not available, the traveling LSI/APOR will be subjected to a facility-based quarantine)

Medical Certificate issued by City/Municipal Health Office at least 7 days prior travel.

Certificate of Acceptance from the concerned LGUs;

Negative RT-PCR Test Result (72 hours validity)

APOR

  • Medical Certificate issued by City/Municipal Health Office at least 7 days prior travel.
  • APOR Undertaking (to be filled up upon entry at the Provincial QCPs)
  • Notice of Coordination from the LGU of destination
  • Negative RT-PCR Test Result (72 hours validity)
